Do you have the logs for a successful boot with uart_2ndstage=1 set in config.txt ?
The '+' symbol is emitted after flushing the UART and is after PCIe reset has completed. It's not clear how far start4.elf has got, it's possible that PCIe did not initialise again within start4.elf
