why is the Fan Not connected to the dedicated header?
do you have other jumper wires (female/female) for making a direct Connection to the Sensor?
have you tested another I2C Interface (i.e. I2C-0 on GPIO0/1)?
have you tested another I2C from your Pi5 (will require external pull-ups)?
