The only one that I can think of is the OpenVMS Files-11 file system. It has versioning so that if you edit a.txt and save it there will be another called a.txt;2. If you want to go back to the original you just use a.txt;1 and when it's saved again it'll be a.txt;3. During some 5 decades of using many different, basic, text editors I don't recall any that change the filename when saving by default. IIRC some may have automatically created a backup file w/o the changes but, usually, a new/different filename required a "save as" action rather than just "save". The only other action i can think of that could produce a new filename is if the "filename" included some form of time/date "stamp".
