OS: I'm not entirely sure right now (I'm not at the machine), but the initial installation of the SD card was done via the "Raspberry Pi Imager" application (I use the "recommended OS" menu option), so it's almost certainly PiOS. The cable idea is interesting, and could be - I'll try a different patch cord.
